Eclinicalworks SQL Query Development

I need a SQL query to extract chronic disease problems and their corresponding SNOMED codes from the Eclinicalworks database. The database is Microsoft SQL Server-based.

Requirements:
- Extract chronic diseases
- Include SNOMED codes with descriptions
