Build me a complex SQL statement

I have absolutely no experience in creating a complex SQL statements and am in need of a top notch SQL expert to create a complex SQL statement that I can use to extract out data. I am attaching a requirements document which lays out the requirements for the SQL statement. Please review and let me know if you have any questions. I need this done as soon as possible please.
