Integration with the backend

I have modified a UI component and added two toggle buttons for 2 new fields in the backend. These toggle buttons are added for a list of names coming from the backend. I need to integrate this with the existing backend api. Most of the code is already written but stuck on a few parts because the form modal is dynamic. Would really appreciate if an expert can fix the code and get it running
