Payment Voucher

I am looking for someone to help create a payment voucher for me that I can use to purchase items and cover expenses. The purpose of the payment voucher is for purchase/expense, and I do have a specific template that I would like to use. Payment will be made with cash. Ideally, the payment voucher should be easy to use and process quickly so that I can get my purchases and cover my expenses with minimal delays.
