Custom Software for Book Formatting and Compilation.

I need a Custom Software Required to Collect the Data from multiple Word/PDF/ files or From a Entry Form and Then Organise it in a Book Format split into two Coloums.
The Application Should Consist of the Following Features:
1. Pre Defined Tempalate according to which the Data will be Set . (For Which Forms will be a Better Option)
2. Editable Option like Line Spacing , Bold Italic Content in the Form .
3. The Application Should be Able to Generate Index based on the Book Made.
4. Option to add / Remove a Particular Entry .
5. Option to Include Unicode / Images / Tables in the Said Form if and where required .
6. Content Can Have Headers , Footers and Footnotes
