Automate filling in a tenancy agreement with signing

I need a app / system property managers where they will fill in the following:

1. Tenant first and last name
2. Id type (Passport / Emirates Id)
3. ID number
4. Move in date / Move out date
5. Rent price
6. Apartment and room number
THEN UPLOAD IMAGE OF ID
THEN SIGN

Once done the doc and ID image should be automatically sent to the specific apartment and room number in the company shared folder. We use drop box / drive.
