An app for TOC implementation

I'm looking to have an app developed for a TOC implementation project. Specifically, I'm looking for a web app that works on the desktop platform. The primary objective of the app is to provide means for close monitoring of pre identified critical business parameters in a company. The app will fetch relevant data from company's ERP and elsewhere, process it, and present it in a summarized form. I require a PHP developer who is expert in both, frontend and backend technologies and also has hands-on experience with MySQL database. Please refer to attached images for more details. The final application has to look like the image App screeshot.jpg and the basic report has to be somewhat similar to the image of an excel file, Output sample.jpg.
