Questionnaire for Application Admins - Assessing Apps for Cloud Migration

I need a questionnaire to provide applications administrators that will allow me to categorize their applications to migrate to the cloud. I have a draft that needs to be updated.
Questions covering the following application architecture, type, hardware requirements, authentications requirements, system interfaces, workload requirements, server/networking, firewall ports, security requirements, encryption requirements, if the application has peak operating seasons, etc. Any additional areas that may be required.
