Need OCR / Computer Vision expert to extract data from PDF - upload to SQL db

Hello,
We're seeking to hire a long term CV/OCR expert who can utilize open source technology to extract fields & values from PDF documents and upload to our MySQL DB table. Fields will need to be standardized from the three different sources we utilize. You can view the sample files attached to this Job post. We'd want to automate the process, perhaps build a script and GUI form where i can visualize the progress and start the processing.
Goals:
1. Read PDF file
2. Extract Fields & Values
3. Standarize Field Names
4. Upload to SQL DB
5. Write script that will automate this process
