Implement a simple feedback form with Salesforce -- 3

Hi! We'd like to implement a simple feedback form (mainly with text & radio buttons) which can be either linked to or implemented from our existing Salesforce framework (existing with custom objects). Looking at the form to be linked to one of our custom objects in SF and a process for automatic email on instruction to email ID of the Contact from the same object. Capturing the response and filling it in back in Salesforce, either in the same custom object or a new one
