Electronic Work Claim Form Transformation

I need my work claim form to be revamped for electronic use, where clients can fill it online and sign it.
To make this happen, I require the following:
- Compatibility: The form should be operational on both the website and a mobile app.
- Signature Options: The form should allow for both typed and drawn signatures.
I have provided my current paper claim form, all fields on the current claim form need to be on the electronic claim form.
I have also uploaded my company logos, letter head, business card logo and current paper claim forms to assist.
