Database Entry and Display Results

I need a software with excel based format to work with Windows 10. User will make data entry of 2 digits numbers with date, numbers from 00 to 99. Based on the data collected and stored in excel format, the software will output 2 results as per photo attached.
Note. The software delivered must passed anti-virus scan and free of malwares such as trojan, phishing etc. The software must be simple to be installed without any assistance or remote access by coder. Therefore, the software must be installed by the user in any computer with Windows 10.
