make an app for subscription based discount card

Health discount card that we sell to our customers and they get discounts from clinics, medical labs, eyeglass shops, dental clinics, physiotherapy etc. The admin enters all the info so it is just a listing. Also we want to make a way so that subscription can be shown from the app so customers can show their subscriptions to service providers to get their discount. An example of a discount is 50% off some lab tests in a medical lab or special discount from and eyeglass store and so on
