Press-1 Autodialer - Custom Solution No Vicidial

I need a press-1 autodialer that places outbound survey calls through my existing PBX over SIP. The goal is simple: play a short recorded prompt, let the callee answer a Yes/No question with DTMF, log that response, and then hang up or transfer if they press 1.
It should have a good UI not very complex. Im looking for partial / ready made solutions. No 30 day waiting times.
