Edit my shopify store to add a custom text field

Hi all,
I need someone to do some edits on my Shopify store to add some custom text fields on the product page. The shopify store sells Facebook advertising for properties via our Facebook page.

Need to add a date option that says:
Open Home time: (only visibly when the 'Open Home' option is selected:
(customer inserts a start and end date)
Custom Target locations:
[ insert a box where customers can type in their custom locations ]
Price options:
$50
$75
$100
$150
There's also some other options I would like added.
