Create custom form for my shopify store
Budget: $30 – $250 AUD
Hi,
I need a form on my shopify store that as the customer fills it out, some of the questions change depending on what the customer answers. I don't want to use an app and would rather it just be coded into my store. I can show you exactly what I need, I have made in photoshop a printscreen of what I'm looking for. I would like this completed today/tomorrow if possible
Here are some measurements for reference
Input box height: 50px high
Box border 0 radius (square edges)
Box border colour #f7f7f7
Box border thickness .5px
Box internal padding 6px 12px
font size 14
font family avenir to match my website
For the check boxes, multiple can be selected at once
The check boxes turn black with a white tick when selected like the photo
The overall width of the form 850px, centred on the screen
Must be optimized for mobile view
In the printscreen please read my notes in blue. Half way down the form, where I ask what kind of project they want to do, wardrobe kitchen etc, if they click something other than wardrobe, the following questions will change. By default it will be wardrobe questions like the screenshot.
I am using testament theme. I think we will need to make a new custom block. Once the customer fills in the form I would like the information to be emailed to me, I can specify the email later.
