Adding custom option to our product page.

We would like 2 custom option on our product page on shopify.
1) Add exactly the same as in picture personalized color options.
2) Seconde is add a conditional option exactly as in picture with custom text on our shopify.
We would like this without using an app.
We really need the design the same as in pictures.
Can you do this ?

We currently are using a page builder (gempages )
