Update Shopify Theme from scss to css & add field to checkout

I am looking for someone to convert my current Shopify website theme from .scss and .scss.liquid to .css or .css.liquid. I am also wanting a dropdown box on checkout saying, 'how did you find us' with the options:
- Google
- Word of Mouth
- Streetwise Pets NZ
- Other
