Shopify website customisation

I need a website building in shopify as same functionlity as https://officialrapidtests.com/
Essentially there are 4 components to the build
1 - User buys the 1 or more products and provides additional data during checkout. they can purchase more than 1 qty (for more than one users).. need some way of customising signup / checkout using things like Userforms to capture data for each user i.e Flight numbers, date of travel etc.. this cannot be captured as a note, rather needs to be data fields. Each qty will need 1 form, so qty 3 is selected, three lots of info needs to be provided
2 - We print and dispatch the items using shiptheory.. it has shopify inetgration of the box
3 - user recieves the test, they login to the system and registers the test.. the data can be captured in a form.. linked to the original order if the qty is 3 and then 3 lots of data needs to be uploaded. i.e. form / product registration repeated 3 times
4 - The data is now in the system, staff member looks in and validates the product registration.. we can categorise this as Positive, negative, inconclusive etc.. upon certain selection (lets say negative) a custom email needs to be sent that has details of the certificate if required including QR code.. if the result its positive then just a generic email is sent
