Shopify Expert!!! To make a changement on my store's cart

I am in need of a real experienced Shopify expert:
I want to use a Form on my cart page, so customers add products to their cart and fill out a form on cart page. Afterwards they send the form and i get all form information AND information about the items on their cart. They are redirected to a success page after.
