Shopify Checkout VAT Relief for Customers

Some of our customers qualify for VAT relief as we sell disability aid products.
We are looking for a solution for our new Shopify website which shows two variations for customers - with and without VAT. *IF* the customer selects without vat, we would need the customer to fill in a form declaring they qualify for VAT relief before they are allowed to check out.
