Shopify: Build a product page that will provide personalized results from a quizz

I need one product page on my Shopify store. Here is the flow of the results expected:

- Customer will feel the answer to a quiz (4-5 questions)
- After filling in these answers, a product with a personalized quantity, and price, will be provided.
- Customers can choose same product (with the same quantity) but with different flavors
- Checkout will include a subscription plan

What I expect is the same as what these websites do:
