Need a custom page in Shopify where users can fill out a form

Not necessarily Shopfiy related, but need a page within my store where users can fill out a form that will then create a custom Pokemon card with the results of the form in real time. When they complete and submit the form, the created card should then be saved in png format and stored in a database (even a google sheets file would be fine) where I can see the created card and then be able to mark it as completed.
