Help us get our Shopify App approved by Shopify -- 2

We are having issues getting our shopify app approved by Shopify repeatedly after numerous submission. We need expert help to see what we did wrong even though we seem to follow exactly their guidelines.
We have 2 issues.
First issue as follow
we are preparing app submission for review, where we encounter error when trying to install the app from the test development store. it appears that there is an extra character appended behind the store name and causing page load error.

there was a extra character appended behind our store name and causing validation error.
