Evaluate Order Form For Shopify

Review, evaluate, and fix code for an subscription sign-up form on Shopify. Form has been encountering multiple bugs and issues that need resolved.
There issues include:
-showing no subscription option available for variations that should have options.
-defaulting to wrong subscriptions when no flavor is selected
-formatting and scrolling issues.
