Custom form on SHOPIFY store

I need a form to be created in between the cart and the payment page. All information in that form needs to be tagged in the order. Here is a website that is doing exactly this. https://www.confirmtesting.com/ go to Fit to fly > PCR fit to fly > Continue and you'll see the form. The information from this form is then transferred to the final order form.
