Create website review contact form in Shopify
Budget: $30 – $250 USD
I am looking for a Shopify expert to create a website review contact form for my website. The contact form should collect the following information:
- Name
- upload button (so customer can upload pics of products)
- Email
- Stars so customer can select how many stars to give product
- content / comment: must have a max of 72 characters.
Customer will receive an automatic reply from us. One that we can customize. This email will contain an automatically generated coupon code (one that shopify will recognize) and good for one use.
To access this form, customer will click a button and it will lead them to a new page that will have a section for the form and three columns for us to create reviews.
