Add form before checkout to shopify website

When the customer is purchasing a specific service on my shopify website, I need them to answer 4 questions before they can make the purchase. Preferably I want those answers to automatically fill out another form on another website which will generate a code which will be sent to the customer.

Questions of the form:
First Name:
Surname:
Date of Arrival:
Vaccinated fully?
