I need help with SharePoint and PowerApps

I need to recreate my intranet in SharePoint and one page in particular consists of about 11 forms that staff can click on, fill out, and then submit. Some forms have an email workflow approval process attached to them and others are simply emailed to the appropriate staff members once submitted. I need help creating list in SP then creating a form associated with it, then perhaps building workflow.
I've attached the current page (ORANGE THEME) along with one of the forms. When a form is clicked, it opens in the blank area to the right. Each form is designed similarly. The goal is to recreate all of them and their functionality in SP.
I've also attached what I have in SP (GREEN THEME) and my attempt with the IT Req Form.
Take a look and let me know if you think you can help. I have a very small budget and need this done by 8/3.
