Sharepoint online visitors logbook Design & Implementation

We'd like to create a visitors logbook in Sharepoint online. It will be multi-lingual (German, English & Spanish: Text will be provided). They should be in a position to check boxes for data protection and EULA before sending the form.
The form should generate a sharepoint list that can later be analysed to visualise the monthly/quarterly/annual visitors. The EULA & Data Protection declaration text can be shown as an overlay for the user to read. All other specs can be discussed in detail later.
It is a simple minor project that should not take more than a few days to implement.
