Sharepoint create form with power automate and flow approvals

Hi I have Sharepoint on cloud, need to create a from with necessary fields with file attachment. Upon submitting, the contents should create items in the list and go for a two step approval cycle with necessary email notification. The last approver should send a comment back to the first approver upon completion.
