SharePoint design

Need I need a SharePoint page design on this page I need a portal log in that has all the documents pertaining to this project I would like it to have questionnaires or reports links available I would like that show what one particular person or crew is doing and how far they've got done I also need it to be able to take what they submit and put it in a chart for project tracking
