SharePoint Project Requirements Form

I'm in need of an experienced SharePoint developer to create a form within SharePoint. The form will be used to gather project requirements and should be adaptable to a template that we will provide. The developer should explain the code so it can be applied to multiple sites and should handle multiple revisions.
Key requirements and responsibilities:
- Design a SharePoint form for project requirements.
- Inclusion of core fields based on the template and instructions
- Adapt the form to include multiple other custom fields as directed by our template.
