Need Dynamic Field customizer of SharePoint List

I need my sharepoint list form to be customizable in terms of it's field labels and field data ...
I mean I should be able to add/delete/modify a field dynamically . field here means field label also
For Ex: If a list has name, date, description fields , User should be able to edit the the field name (for ex can change name to username run time) and can add/delete more columns dynamically.
