Door Register / Inspection Form Microsoft Power App

I'm working with a client placing QR codes on doors in blocks of flats (apartments).
They need an app with two forms:
Form 1: Record where they have put a QR code and record static information against that door.
Form 2: Record inspections agasint these doors on an ongoing basis. Inspection history needs to be archived.
The app would have the functionality to scan QR codes as part of the forms.
The ability to save pictures of doors. (linked to specific inspections)
The app can initially have data within SharePoint, potentially azure if required.
I have attached questions for the initial creation of the door/QR record. And inspection questions.
The inspection form should be separated into separate form pages for ease of use.
I can host a prototype and will need to transfer it to the client's Microsoft environment.
