Custom Power Apps Form for Daily Reports

Description:
We are seeking an experienced Power Apps developer to create a custom form that will streamline our daily reporting process and integrate seamlessly with Excel. The form will be used to collect and manage project-related information from our subcontractors and ensure all data is organized and easily accessible.
Project Requirements:
Form Layout:
At the top of the form:
Date Picker: For selecting the date of the daily report.
Subcontractor: A ComboBox populated by a SharePoint list.
Email, Telephone: Text fields that auto-fill based on the selected subcontractor from the same SharePoint list.
Project: A ComboBox populated by a project SharePoint list.
Project Number, Project Address, Project City, Project Postal Code: Text fields dependent on the selected project.
At the bottom, I need a gallery with 5 columns:
Activity: Populated from an activity SharePoint list.
Activity Code: Dependent on the selected activity.
Type of Work: Fillable field.
Description of Work: Fillable field.
Square Footage: Fillable field.
Data Export:
The form should export data to Excel with each row containing the following fields: Activity, Activity Code, Type of Work, Description of Work, Square Footage, Date, Project Number, Subcontractor Name.
Key Skills Required:
Proficiency in Microsoft Power Apps and Power Automate
Experience with SharePoint integration
Strong knowledge of Excel and data export functionalities
Attention to detail and ability to create user-friendly interfaces
Bonus:
Create Item on Board in Monday.com
Once Item is created, use Item ID to create subitems (each row = 1 subitem)
