ServiceNow Now Mobile Sign-In Pages

I need a simple, corporate-style ServiceNow Now Mobile and on now Agent built in a gray-and-black palette. The landing page after login shows a brief welcome message followed by three clear buttons:

• Visitor Sign In
• Contractor Sign In
• Log Out (must log users out of the NOW Mobile app)

Workflow requirements

Visitor Sign In
1. Redirect to an information page with the same header styling and two buttons: “Do you agree” and “Do not agree.”
2. If the visitor agrees, take them to a short form capturing Name, Email, and Phone Number.
3. On submit, create a corresponding record in the target table.

Contractor Sign In
1. Follow the same agreement step.
2. If agreed, display a form with Name, Purpose of Visit, and Date.
3. Save a new record to the same table (or a separate one if you prefer—just note it in your build notes).

Technical notes

• Build natively in ServiceNow; no external hosting.
• Include a basic readme so I can import, test, and extend it later.

Deliverables

• Exported update set (or scoped application) containing the landing page, agreement page, two form pages, client scripts, and table configuration.
• Brief setup instructions and screenshots confirming record creation and successful logout behavior.

This is an initial, lightweight build; a straightforward, functional prototype is all that’s required right now.
