Edit self report widget OOB Widget to add Start date of Isolation

We have implemented safe workplace application in servicenow,
Just want to add One field (Start date of isolation) on OOB widget to show to the end users.
OOB Widget name : Submit your Health Status ( App: Emergency self report)
Just need to add one field so user can Insert their input.
I am not familiar with the widget so need just small guidance and help.
I dont think it will take more than 1 hour for exeprience portal developer.
