Enhance Python Form-Filling Automation

I already have a working Python script that automatically fills out online forms for routine data-entry work. Since the website’s layout and validation rules have recently changed, the code now misses a few fields, throws occasional errors, and needs a general cleanup.

What I need now is a concise yet robust refresh of that script. You’ll review the existing codebase, refactor it for clarity, update the selectors and field mappings, and add solid error-handling and logging. If you prefer Selenium, Playwright, or a similar headless-browser tool, that’s fine—just keep it in Python and make sure any new dependencies are noted in requirements.txt.

Deliverables
• Updated Python script with all form-filling steps working end-to-end
• requirements.txt and a short README covering setup and usage
• A sample run (log file or short video) proving the form is completed successfully without manual intervention

Acceptance criteria
• Script completes the full form using test data I’ll supply, with no unhandled exceptions
• All field values appear correctly in the confirmation screen
• Log file clearly records success/failure for every submission attempt

Please keep the code readable, modular, and ready for future additions such as spreadsheet updates once this piece is stable.
