Docusign - form automation

Problem description:
There is an online PDF form (created with docusign) that I need to fill out to many times.
It must be a better way then just type all this info again and again.
I tried a few plugins / Selenium Browser Automation but without success.
Problem is that each time form is generated it has unique id / css on input.
Solution:
Looking for some automated script that could read CSV file and fill the form.
