Square Space - Salesforce Web Form Integration

I need to have a form on my Square Space website integrated into our Salesforce system. When a lead completes a form, the data needs to route into Salesforce as a lead and we need email then to be sent to the subscriber from Salesforce.

Please advise of cost, any requirements (extra software) and time to complete.
