Connect CRM To Consult

PLEASE READ COMPLETELY: i need someone who can connect my consultation form on my site https://leihomeenhancements.com/ to my CRM (salesforce) https://www.salesforce.com/
here is the exact thing i want done when someone submits a request for a consultation they are sent to a certain team based on the customer state/zip code - at that point any person from that team can reach out to that customer and move them up in the pipeline rather that means they are now a hot lead (interested in someone coming out and giving them a consultation or they are not in the price range to be able to get/afford the services - you will also be in charge of creating the pipeline - you must either be aware of how to create and use a pipeline in salesforce - im unsure what language my site is written in so hopefully your knowledge can determine that - i need someone who understands what needs to be done and how to do it please use the phrase "CONSULT FORM" please explain how much you will charge for what i need and how long it will take please ask any questions that you need to know tobe able to give me an accurate price do not ask me my budget based on the information i have provided give me your price
