Access Data to Salesforce.com for use with SPIFF app

We use sf.com and have purchased SPIFF which has integration with Sf.com. The data needed to populate the custom SPFF data tables in sf.com lives in our home grown access db QUote to ORder tool. It's very mature tool but is not fully documented, the data we need to get from quote to order tool are as follows
Per our call today, I've compiled a list of ALL the data Spiff is using from Salesforce (see attached). I believe Michael's process only involved uploading data into the Spiff__c custom object, but it is possible he had steps that impacted other objects so I wanted to include everything we are touching to be safe. I also included fields that are generated by Salesforce (Id, Created Date, etc) but those aren't fields you would need to populate when creating records.
After filtering out other objects and system fields, here is the data you will need to extract and import:
