API to Salesforce assistance for a website with forms that are sending data to Salesforce

Our client is biospine.com. We need to duplicate a form and then have the data and form process the same as it does when you use the form that is on the sidebar. This form has multiple steps based on what you choose for "Type of Request."
We already have forms sending data to Salesforce using API codes, the person doing that is no longer available but we have a developer on our team familiar with API, we just need some guidance on where the functions are located and how they work. We would like to go through the process of duplicating the sidebar form and having it act the same way as the original form with you so that we may learn how to do it properly.
