Ruby on rails form pages update

Hi, i have old looking sign up/ sign in / rest password..etc pages built using ruby on rails "extension html.erb". i have bought these new good looking HTML 5 CSS forms https://codecanyon.net/item/appforms-modern-responsive-forms/21622372 , and i would like to make them usable to replace the old pages
