Risk Assessment Matrix Creation

I need two concise 5 × 5 matrix risk assessments completed on the template form I have supplied (an example is attached for style and level of detail). Both assessments must cover a specific task with its associated risk, each task must have some realistic level of risk of injuries to staff, students, and guests—trips and falls included. The second should address possible fire safety risks across the school premises associated with a task, for example, a fridge compressor change, which requires welding, so possible fire risk etc.
Both assessments must be written directly in the provided form, following its layout and wording exactly. Each matrix cell should be scored, control measures listed, and residual risk recalculated so that anyone reading it can digest the result in under a minute.
