Questionnaire construction ( quantitative + qualitative )

Hi i am looking for a freelancer who can construct a questionnaire on the topic Solid waste management in a city.
The questionnaire should have two Parts
Part- A
Household Survey
(quantified info)
Part- B
Construction of qualitative questions- FGD/semi structured interview to be asked to stakeholders.
The whole questionnaire should be of 10 pages with combination of questions( Likert scale questions/open/with answers)
The freelancer will have to study the available questionnaire and some literature to construct a new.
I want a freelancer who ask minimum questions and manage the project with his wisdom
Deadline is- 4 days
