In the US, there are healthcare providers' associations (e.g., for oncology, there is ASCO; and then there are the organizations listed here: https://www.accc-cancer.org/state-societies and others not listed there, such as www.msho.org).
I need to build a list of State-level physician associations in the US, as a table (table specs below). Rows should contain State / Specialty / Name of organization / URL / Contact Phone number. Some States will not have an association; some associations might span multiple States.
The specialties I would need to build that for are:
* Neurologists
* Occupational Therapists
* Physical Therapists
* Dermatologists
* Nephrologists
