I need possibly a forensic psychologist

Am moving to Israel and one of the immigration agencies in that country is requiring that I obtain an evaluation. This evaluation is to determine my abilities to travel internationally and adapt to a different living environment. The question came up when I filled out a health questionnaire and stated that I have OCD. Fortunately, it is being treated successfully and I do not participate currently in any psychiatric or mental health treatment.

I am seeking a letter which is to follow the guidelines in the attachment herein.
