Urgent Motion to Compel Filing

I need help with court protocol and procedure file a Motion to Compel on my behalf. I can write my own motion, just need to know I am using the right form, providing the required artifacts, filing properly and serving properly so that I can get the matter before the court. The situation is urgent as the child has become suicidal.
The purpose of this motion is to enforce compliance with a prior court order regarding the parenting plan.
